Novant Health North High Point Family Medicine welcomes new patients! Whether you need an annual physical exam or help with an acute illness, our providers are here to see you quickly and at a a time convenient for you.

Schedule your first appointment anytime online using MyChart or by calling our office during regular office hours. Let us know if you or your child are sick and would like to be seen quickly.

Bring with you to every appointment:

• A photo ID.
• Your insurance card.
• Your method of copayment or payment in full if you are uninsured. (Click here for insurance information.)
• Any medications, including over-the-counter medicines and supplements, you are currently taking.

Please arrive 15 minutes before your first appointment so that we can collect necessary information from you. You can also download and fill out forms ahead of time by clicking on the links below.
